Employees have the opportunity to reduce health insurance premiums by participating in a two step health risk assessment. All information is confidential, adhering to HIPAA and federal guidelines, and is for employees to use for health maintenance and preventive measures.

BOTH STEPS ARE REQUIRED TO RECEIVE THE DISCOUNTED PREMIUM.

 

STEP ONE-- Must complete by Nov. 14, 2008. NO EXCEPTIONS.

This part of the assessment involves a biometric screening for blood pressure/pulse, cholesterol, glucose, height, weight, and body mass index. You will need the results of the biometric screening to complete step three.

You need to choose either option one or option two. You do not need to do both options.

   

  > Option one: Go to your primary care provider.

  

  > Option two: Go on-campus (by appointment only).

 

STEP TWO-- Must complete by Nov. 17, 2008. NO EXCEPTIONS.

Complete the online questionnaire. Marquette University has partnered with the University of Michigan to offer a comprehensive Health Risk Assessment process for you to evaluate your health using a combination of a biometric screening (step one) and an online questionnaire (step two).

The online questionnaire will ask for your Social Security Number. DO NOT enter your Social Security Number.  Instead, enter your Marquette University Employee ID number. (Example: 000001234; must be nine-digits in length). 

Type in the Group Password = MARQUETTE

Type in HRA Password - Create your own password and remember your password for next year!

Click on gender type and answer the smoking question.

Click on Enter.

Complete questionnaire and print out report. 

Please note that you cannot change anything or go back into the questionnaire once you have clicked on "Submit".
