Employees have the opportunity to reduce health insurance premiums by participating in a two step health risk assessment. All information is confidential, adhering to HIPAA and federal guidelines, and is for employees to use for health maintenance and preventive measures.
BOTH STEPS ARE REQUIRED TO RECEIVE THE DISCOUNTED PREMIUM.
STEP ONE-- Must complete by Nov. 14, 2008. NO EXCEPTIONS.
This part of the assessment involves a biometric screening for blood pressure/pulse, cholesterol, glucose, height, weight, and body mass index. You will need the results of the biometric screening to complete step three.
You need to choose either option one or option two. You do not need to do both options.
> Option one: Go to your primary care provider.
- Biometric screenings are valid July 1, 2008 - Nov. 14, 2008. Results must be faxed (contact information below) to Aurora Health Care by Nov. 14, 2008. No exceptions will be made for forms received late. Your visit will be subject to any applicable copay.
- Download, print, and complete section one of the Health Risk Assessment Biometric Screening Primary Care Provider Form. Ask your primary care provider if they prefer receiving the form via fax, mail, or in person. Ask for a copy of your biometric results, as you will need the results to complete step three.
- You or your primary care provider must fax the completed Health Risk Assessment Biometric Screening Primary Care Provider Form no later than Nov. 14, 2008 to:

RESULTS SHOULD NOT BE SENT TO MARQUETTE UNIVERSITY
> Option two: Go on-campus (by appointment only).
- Free biometric screenings (copay waived) will be available on campus Oct. 27 through Nov. 14.
- Oct. 6 - Nov. 13 - Call (877) 765-3213 or register online to schedule an on-campus biometric screening at the Alumni Memorial Union.
- Bring your MUID to your screening.
- All forms will be provided to you at the biometric screening.
- An Aurora staff member will give you the results of your biometric screening for step three.
RESULTS SHOULD NOT BE SENT TO MARQUETTE UNIVERSITY
STEP TWO-- Must complete by Nov. 17, 2008. NO EXCEPTIONS.
Complete the online questionnaire. Marquette University has partnered with the University of Michigan to offer a comprehensive Health Risk Assessment process for you to evaluate your health using a combination of a biometric screening (step one) and an online questionnaire (step two).
The online questionnaire will ask for your Social Security Number. DO NOT enter your Social Security Number. Instead, enter your Marquette University Employee ID number. (Example: 000001234; must be nine-digits in length).
Type in the Group Password = MARQUETTE
Type in HRA Password - Create your own password and remember your password for next year!
Click on gender type and answer the smoking question.
Click on Enter.
Complete questionnaire and print out report.
Please note that you cannot change anything or go back into the questionnaire once you have clicked on "Submit".
