University Policy and Procedures (UPP) 1-18 delineates the requirements and guidelines for all Marquette employees who organize and or participate in travel abroad involving participation of Marquette students. This policy covers all service learning programs, service projects, student organization sponsored travel abroad, service trips, study abroad programs, international programs (including the Les Aspin Center) and any other trips involving Marquette employees traveling abroad with Marquette students participating.
REQUIREMENTS
- Policy: UPP 1-18
- Employee Foreign Travel Information Form 12/04 [employee traveling internationally involving student participation] (completed by every Marquette employee participating in international travel with students participating)
- Employee Foreign Travel Information Form 8/03 (completed by every Marquette employee participating in international travel. If students are participating form 12/04 should be used instead of this form.)
- Waiver of Liability and Release Form (contact General Counsel 288-7343) This form must be signed by every student and every non-Marquette employee participating in the trip
- Trip Participants Contact Information Form(completed by Marquette Employee coordinating the trip)
- Emergency Contact/Medical Information Form (completed by every trip participant)
