- Three letters of recommendation
- Personal statement and curriculum vitae
- Undergraduate and dental school G.P.A.s and class ranks
- Scores from National Board Dental Exams
- (International Resident/Students) TOEFL score or other acceptable proof of English proficiency*
*All foreign trained dentists that are not already licensed to practice in the U.S. are required to take a preclinical dental proficiency exam as a requisite for acceptance.
