University Policy and Procedures (UPP) 1-18 delineates the requirements and guidelines for all Marquette employees who organize and or participate in travel abroad involving participation of Marquette students.

REQUIREMENTS
All forms must be submitted to the Office of International Education (AMU, 407)
- Employee Foreign Travel Information Form 12/04
Employees traveling internationally involving student participation - to be completed by every Marquette employee participating in international travel with students participating.
- Waiver Request Form
This form must be submitted by the program faculty/staff leader at least 8 weeks prior to departure. This document will be used to produce the waivers of liability and release which must be signed by each participant at the pre-departure orientation.
- Program Participant Information Form
To be completed by each participant.
- Program Itinerary Form
To be completed by Marquette employee coordinating the trip.
- Faculty Emergency Contact/Medical Information Form
To be completed by all faculty participants.
- Student CISI and Medical Information Form
To be completed by every student participant.
