Notetaker Request Form

If you are in need of notetakers for your classes there is a form you need to complete.  This is the Notetaker Request Form. On this form, you list your classes, section, and days and times the classes meet.  Notetakers will be sought from your classes to provide you with notes.  You will also need to complete a Notetaker Contract, which is simply an agreement indicating that you understand how the process works and your responsibilities as a notetaking recipient.  The Coordinator will review the form with you when you meet to discuss your accommodations.  The Notetaker Policy provides more details on how the process works.
