Housing Requests; Students with disabilities who perceived their health, mobility, or other disability issues may impact their ability to equally access the services or programs in their residence hall may request special housing accommodations to mitigate the impact of their disability.

- Special Housing Requests as an Accommodation

For currently enrolled students:  If you've been approved for back-up notetakers, textbooks on CD, test proctoring assistance from ODS, and/or extended test time, each semester you desire these services, you will need to fill out the appropriate request form and return it to ODS. 

You may pick up copies of these request forms from ODS (AMU 317) or you may print them from the web where they are available in PDF format.

If you need help completing any forms, come to ODS in AMU 317 and we'll be happy to assist you.

For students and their treating professionals:  ODS may occasionally require more specific documentation of your disability from your treating professional.  To assist your treating professional in providing the specific information necessary for documenting your disability, click on the link to the PDF version of the appropriate certification form, print it and forward it to your doctor, psychiatrist or other appropriate professional. Completed forms can be dropped off, faxed to (414) 288-5799, or mailed to Heidi Vering, Coordinator, Office of Disability Services, Marquette University, P.O.
