Do I need to complete the Health History Form?

 

All newly matriculating students are required to complete an Immunization Record, Tuberculosis Screening and Medical History Form.  These forms can be accessed online from the Student Health Service web site, or picked up directly from Student Health Service. When you have completed filling out the form, please drop it off at Student Health Service or mail it to:

When we receive your form, it will be regarded as confidential information and will become part of your medical record at Student Health.

Failure to submit the required documentation within 30 days of the start of your first semester/term at Marquette will result in a medical hold, preventing future registrations. This hold will be removed once the immunization and TB screening requirements have been met.
