Earning Rewards

You are eligible to receive a punch on a Wellness Rewards Card every time you:

• Attend a qualifying employee wellness program
• Get 20 or more checks in a month on your Healthy Living Calendar

Complete your punch card by earning 10 punches in any combination. Each completed card earns you a Wellness Reward Package and entry into an annual Grand Prize drawing!

Receiving your Reward Card

Wellness Rewards Punch Cards are available at all qualifying employee wellness programs. You may also request one by contacting Mandi Richter, Wellness Coordinator, at 8-4581.
