Description of Agency: Community Care is a model of long-term managed care for the frail elderly, one of only 16 similar programs in the country. The mission of the program is to provide all-inclusive care in one place and to allow participants to remain independent in the community, thereby avoiding premature institutionalization (e.g. in nursing homes). Care is managed by a multi-disciplinary team (social workers, nurses, nurse practitioners, doctors, occupational therapists, physical therapists, recreation therapists, etc.). This team creates a plan of care for each participant. Care may be given at the sites or in the homes of the elderly recipients. There is also a social aspect to the activities at the centers. The Prospect Ave. site has a residential component. This is known as "assisted living" (as opposed to the skilled care provided in nursing homes), where residents receive limited assistance and some meals.
Student Role in Agency: Service may be tailored to meet the needs of both the MU students and the participants. Some possibilities include:
Commitment: Orientation, plus a minimum of a 2-hour block of time per week. TB screen questionnaire to be filled out at site. TB test not required for this site. Students must be on time each week and call ahead of time if an emergency occurs and the student is no longer able to attend his/her service hours.
