Green Sign-Up Sheet

All Service Learners must complete a green registration form prior to the start of their service. The "Green Sheet" is used to record Service Learners' contact information, what placement they are interested in, and what course they are doing their service for. The information is logged into the Service Learning database. This form is kept on file for five years in the Service Learning Program Office for reference purposes. Green Sheets should be handed in immediately after signing up at Sign Up Night or directly to the Service Learning Office.
