Marquette Incident Reporting

To report non-emergency incidents for review by university staff, please choose from the following submission forms:
- Standard Incident Reporting Form — Used to report non-emergency incidents, safety concerns, or violations of university's Community Expectations that may require review or follow-up by university staff.
- CARE Team Referral Form — Used to share concerns about a student’s well-being so the CARE Team can assess the situation and connect the student with appropriate support and resources.
- Title IX Reporting Form — Used to report concerns related to sexual misconduct, sexual harassment, dating or domestic violence, stalking, or gender-based discrimination under Title IX protections.
- Bias Incident Reporting Form — Used to report incidents of harassment, discrimination, or concerning behavior motivated by bias against a person or group on the basis of a protected class.
- General Student Complaint Form — Used to raise concerns about a university process, service, or experience that does not fall under other specific reporting categories.
Submissions to these forms are reviewed daily during the week. Staff may follow up with reporting parties based on the situation, but they are limited in what they can share about any university actions due to federal privacy regulations.
