All students are required to complete the Immunization History and Tuberculosis Risk Questionnaire on the Marquette University patient portal.

Proof of immunization is required for:

• MMR (measles, mumps, rubella)
• Varicella (chickenpox)
• Tetanus/Diptheria/Pertussis
• TB risk questionnaire (located under the forms section in the Marquette University patient portal)

Failure to provide proof of immunization within 30 days of the first session of enrollment will result in a medical hold preventing future class registration.

Important Notice:  If you are a student in a health profession, it is important to confirm additional health and immunization requirements.  Please consult with the Dean’s office or the program’s student handbook.

*Exemption from immunization compliance for students in FULLY ONLINE programs, no residency requirement or other program events requiring a campus visit, maybe granted.  Contact the Program Coordinator of the program for which you are enrolled or the Marquette University Medical Clinic.
