COVID-19 Employee Requests for Temporary Alternative Work Arrangement


Marquette University intends to resume on-campus operations in fall 2020 if medical and government authorities deem it safe to do so, and with the proper health and safety measures in place to help lessen the spread of the coronavirus. Employees may be seeking alternative work arrangements due to medical or personal reasons relating to COVID-19. Marquette University will determine the availability of temporary alternative work arrangements based on college/business needs of the University, and in consideration of the essential functions required for specific jobs/positions. Temporary alternative work arrangements may or may not be fulfilled as requested; where appropriate, an interactive process may determine other options. Any arrangements made will be approved for a specified duration and subject to reassessment to ensure ongoing effectiveness.  


To request a COVID-19 temporary alternative work arrangement, please complete the request form below and submit it to HR. If you are seeking an arrangement due to medical reasons, you must also complete and submit the Medical Information form, which requires medical provider documentation. Your medical information will remain confidential and will not be shared outside of Human Resources.

Form 1095-C is a tax form reporting information about an employee's health coverage offered by Marquette University. The taxpayer does not fill out the form and does not need to file it with a tax return. It should be kept with the taxpayer's records. For additional questions on this form, consult your Tax Advisor.

The 1095-C form is being sent out to the address you have listed in MyJob.  If you need a copy of the 1095-C form, you can access it by logging into the enrollment portal and follow the instructions on this link.







Per the University announcement on June 22, 2020, the employer matching contribution to Marquette’s 403(b) retirement plan will be suspended for the fiscal year 2021.  HR has compiled some frequently asked quetions regarding this change.
