- Forms for event registration
Eventbrite can be used for registration for paid and free events. The setup allows for custom fields and registration settings, including limiting the number of tickets for an event and multiple ticket types (free, early bird, student tickets, etc.). Also allows for mobile ticketing. The tool is free to use, however there are fees for paid events.
- Forms that allow document uploads
Qualtrics Survey Forms
Qualtrics is ideal for surveys and feedback forms, and also works for more basic inquiry forms. Access Qualtrics with your Marquette account to author surveys. Forms can be public or private, and private forms can be password protected or accessible by invite only. As of Sept. 17, 2018, Qualtrics started offering file uploads.
- Basic inquiry, contact forms etc. (For users whose sites are in the OU Campus CMS)
OU Campus Forms (CMS)
OU Campus Forms can be used to create simple forms such as contact forms and inquiries. Your website must be part of the OU Campus CMS in order to use this tool.
- Microsoft forms may be used if you want to limit access to specific Marquette users and require their Marquette login information to view the form. For more information, contact the IT Services Help.

Health information and student privacy protection
Make sure no one is entering health information or protected student information into third-party forms. View Marquette’s guidelines on HIPAA and FERPA.
As we have been the target of spam attacks, the form must use a reCAPTCHA or similar spam protection that can differentiate people from robots.
Over half of all web traffic takes place on a mobile device. To account for this, the form must be accessible with a mobile device. Test on your smart phone or use an emulator on your desktop computer.
We are required to meet Web Content Accessibility Guidelines (WCAG 2.0 AA) to make sure our sites are usable by visitors with impairments. You can use their online tool to check your form for issues.
If the form is for an official Marquette event or will be placed on a marquette.edu website, OMC needs to review it before it goes live. Please send to email@example.com.
