Endodontic Referrals

PreDoc Referral Form - Department of Endodontics

PreDoc Referral form uses:  Anterior and pre-molar teeth ONLY referred for initial treatment may be evaluated/treated by 3rd or 4th year dental students under faculty supervision.  If the patient needs to have a post and core and/or post space, then the patient will ONLY be considered for treatment by an Endodontic Graduate Resident and NOT pre-doctoral students (A Graduate Resident Referral Form should be used).

Graduate Resident Referral form uses:  All teeth, post and core and/or post space work is completed by a Graduate Resident.  Please note that if you are referring for an anterior or premolar initial treatment, wait-times are much shorter if you choose the Predoc Referral.

Trauma Referral Form - Department of Endodontics

Trauma Referral form uses:  For health related injuries and/or injuries to the tooth pulp (the soft inner tissue), gums or surrounding tissues often resulting from accidents or severe tooth decay.
