How to Become a Patient

Marquette University School of Dentistry is committed to protecting the health, safety, and well-being of our students, faculty, staff, and visitors. We are carefully monitoring the developments related to the Coronavirus outbreak.

Nevertheless, COVID-19 is a highly contagious virus and the danger it can pose to vulnerable populations demands that we take precautions to protect all members of our Marquette community.

Our Graduate Specialty clinics opened on June 15th and our Pre-doctoral clinics opened on July 13th.

From July 20th onwards, normal hours will be resumed at the Emergency Clinic and the clinic will be open Monday - Friday (8:30 AM - 12:30 PM AND 1:00 PM - 4:30 PM).

Our current patients with a dental emergency can call (414) 288-7388 for additional information.

After hours protocol will be in place during the time the Emergency Clinic is closed. When the after-hours protocol is in place patients can call (262) 521-4997.

There will NOT be walk-in emergency services available for the community until further notice. We have a dedicated team working through these complex issues to identify the best course of action in the coming days and weeks.



Making an Initial Screening Appointment

  • Call (414) 288-6790, Monday through Friday, between the hours of 9 a.m. and 4 p.m. to speak with a representative to schedule your initial screening consultation.
  • Please provide the representative with your email address so you can register and fill out your health history in the MyDental portal.
  • For children under the age of 13, call our Pediatrics Clinic at (414) 288-7273 to schedule an appointment.

Before Your Screening Appointment

  • If you have provided your email address, you can fill out the Health History online by creating your MyDental record. Once you are on the MyDental portal, select "Register for Access" to set up your account.
  • Click here for printable instructions on accessing MyDental.
  • If you do not wish to create a MyDental record, please contact and we will send you a PDF version of the health history form. Print and fill out a copy of your health history and bring it with you to your screening appointment. Please indicate in the email the type of health history form you require. For example: 
    • Adult - English
    • Adult - Spanish
    • Children - English
    • Children - Spanish
  • You will also be given time to fill out the health history form on the day of your screening consultation as well.
  • Please bring the following information with you:

    • Medicaid/ ForwardHealth insurance card
    • Any private dental insurance cards
    • List of medications
    • Physician information, emergency contact and, if the patient is under 18, the parent or guardian's information.
  • Please make arrangements to leave ALL children, infants, pets and dependent adults at home. They may not accompany you to any dental visits.
  • All patients under age 18 need a parent or legal guardian to be present during the appointment to provide signatures on consent forms.

Screening Cancellations

  • We require 24-hour notice for all cancellations. Please call (414) 288-6790 to cancel your screening appointment.

Treatment will discontinue for patients who fail to keep appointments without prior notification, are consistently late or cancel more than two appointments.
