Confidentiality: Please do not inform the nominee they are under consideration since only a limited number of recipients are chosen each year from among the many worthy nominations received. All deliberations are confidential.
Eligibility Criteria:
Each recipient is expected to attend the awards ceremony to accept his or her award, except in extraordinary circumstances.
Please submit a separate form for each person you are nominating. Submit one form for couples or a family nomination.
While letters of recommendation and extra materials aren't required, they help the committee see the full scope of the nominee’s impact. The quality of the information provided is more important than the quantity or relative prominence of the writer(s).
